This appears to be common behavior in any bureaucracy where the solution is too complex and/or time/money consuming to accomplish, so everyone involved just accepts an obvious loophole and calls the issue solved when it's not.
It's a combination of laziness and ineptitude that only goes under the radar due to a lack of visibility on just how frequent this occurrence is.
