You dont need a note, just tell everyone you have an over active frontal cortex that requires you to get the correct amount of oxygen at all times.

Specifically, my dentist, yes the person sticks her face in my mouth, requires one while sitting in the waiting room. I may or may not have called her receptionist a cunt.

just tell them you cannot wear a mask because you've been diagnosed with a severe case of CSD (common sense disorder) - they won't know what the fuck you're talking about and may very well let you pass :)

if that fails, tell them you'll be boycotting their store

Make a fake one

I would assume it has to be on letterhead or a prescription pad. I won't know. Maybe there's a form online.

if any worker from any establishment asked to see a note: first, they're no medical professional, so their opinion on what is good or not good is null and fucking void. Second, if they ask for your personal medical history as a term of service, in a 'professional capacity,' I would consider it a HIPAA violation and consult counsel immediately to see where you stand.

medical facilities are enforcing the mask rule pretty heavily due to potential liability suits - even chiropractors. funny thing is, they have you remove the mask so they can get on with whatever procedure you're there for, defeating the entire purpose.

The CDC website says that even if both people are wearing a mask, a person is at high risk of infection if contact with an infected person is more than 15 minutes.
