Grab yourself by the balls, using both hands. Now twist your sac in the same direction with both hands. Then twist your balls in one direction with the top hand and the opposite direction with the bottom hand.

The crazy thing is the guy obviously moved the bottle clockwise. It's almost like he was trying to move the cap Counter-clockwise or something.
