We had a guy in our shop once start to put WD-40 on the threads of the gauges when swapping out an oxygen tank for the acetylene torch. He almost got fired over it.
The magic of oxygen is that it turns EVERYTHING into a boom device. Normally harmless combustible objects like a stick of wood or lump of charcoal turn into sticks of TNT when soaked with O2. It's also the same reason why ammonium nitrate NH₄NO₃ is a favorite of backyard chemists - comes with lots of its own oxygen.
That poor kid never had a chance in a bubble full of oxygen, and everyone handling that situation should have known better.
