One of the differences is that if you drink next to someone, only you suffer the effects of the alchohol. If you smoke next to someone, their health is directly affected by your smoke.

If someone sits besides, and you don't like the way he looks or smells or whatever, you get up and move away....and these days where the hell can you be, that a smoker can sit next to you? Bar? Hell no! Library? Hell no! Store? Hell no! Office cubicle? Hell no! The anti-smoking assholes have banned cigs pretty much everywhere, and are working feverishly on banning it in apartment buildings here (so one cannot even smoke in one's own damn apartment).

Alcohol is worse all around. Drunk driving injuries and deaths is an example of second hand effects, domestic violence, broken families, and cost on our health care system are others.
