So was this never equipped, equipped and removed, or equipped and not working? Equipped but never worked?
I find this admission to feel misleading for some reason. It's going to come down to systems and practices. When we get to "clerical error" you'll know that it's just people writing down that they do the work but don't actually do the work... This is where it becomes criminal.
