110lb cardstock, 4.25 x 3.25, laser printer, keep the card in your pocket for a day or so(rough it up), then photocopy it, poorly, and destroy the original. Search your local news for when a pop up vax site was setup for dates on card...call it a day. Try to find a corresponding Johnson and Johnson vax because it's just one shot... As of yet, there is no database to verify your card data. All anyone wants is to see you hold up your paper proving you complied. All of my fishing buddies are doctors. Not one has taken the vaccine, nor suggested it to anyone.
That all being said, do you want to work for someone that would force you to do something medically in order to work? Is losing your dick and balls next??
That being said, your employer is being forced to ask you to vaccinate... print and done, issue resolved.
don't forget to destroy the original.

"What if I lose my card?"
"Try not to lose your card. Keep it in a safe and secure place that you’ll remember until the time of your second dose. Tip: consider snapping a photo of the card on your smartphone once your first dose is complete. Making a photocopy is another option."
So, apparently just a picture of your card on your phone is sufficiently theatrical to satisfy.
"Where's you original card?"
"Stored in a safe place in accordance with the published guidelines."
"Well where is that?"
"Well, not that it's any of your business, but it's locked safely away in the back of the bottom drawer of a large gray steel filing cabinet, in a steel lined vault, in the basement of the Banc Suisse, in Zurich. Right behind the sign that says, 'Beware of Leopard'. Happy?"
