I think this has it:

Clearly unbalanced, then tried using algorithms to rebalance, but the algorithms were not tuned which led to wild oscillations, furthering the imbalance. This is not an “attack”

I think being suspended from the hook screwed it up when it was trying to balance on its feet. It wasn’t trained for that.

On one hand, it's amusing, but given the state of contemporary engineering, this is very plausible. Consider the QA testing that goes on in industry... exactly. We all know what happened in program warp speed. We're still experiencing the after shocks. Of course, no one is held accountable... oh wait, the entire industry had immunity of liability. We need to re-think that strategy.
