Someone brought a real and loaded gun to a movie set, it somehow got mixed in with the props, the handler didn't check it properly before it was handed to Alec, and he pointed it at the head of someone who is not in the movie, and pulled the trigger. Mistakes were made. No big deal.

I've heard that they were told to do the scene again. At this point, he pointed the weapon at the director and fired.

It is clear to me, no one on that set should have been handling a weapon.
