It’s suspicious, but no admission of problems yet. Someone called the number (twitter.com) and all they tell you is that you have to wait two weeks after getting an attenuated virus COVID-19 vaccine, which is not offered in the US anyway.

It’s odd because they could have simply said that in the questionnaire. The wording is also odd:

Have you EVER had a Coronavirus (COVID-19) vaccine?

Why does it say “EVER” instead of “in the past two weeks”?

It looks like they are concerned about vaccinated blood, but they’re not actually refusing any donations.

Maybe a few years from now they will be testing all vaccinated blood and this will be seen as an early hint they were going to do it.

I can’t see them ever flat out refusing blood donations from all vaccinated people because that would be a disaster. They would have the same number of people needing blood but only ~40% as many eligible donators.

The "have you ever" could be explained by the general stupidity of people and the attempt at covering all use cases.

If I say "have you, in the last two weeks, had an attentuated virus vaccine injection for Covid-19" - that's going to make the sheep's eyes glaze over while their brain melts. The "have you ever EVER" covers everything and is easier for an idiot to digest.
