An 18 hour shift seems outrageous to me. There is no way that the cognitive ability is fully functional after 17 hours of working

Welcome to the world of doctors. 24 hour shifts are not unheard of.

It is what it is. It's actually worse when you're still learning your practicum. You'll be on-call/on-duty for days and sleep at the hospital when you can find room to sleep. That's after your 8 years in college.
