“We are at a turning point”: The coronavirus outbreak is looking more like a pandemic

Health experts say it’s time to prepare for worldwide spread on all continents.

Marlene Millen, MD

Given multi country sustained spread occurring it is important to understand what may come next with #Covid_19 and continue #pandemic planning in healthcare.

CDC added a new section to this update.

What May Happen Next in the USA:

More cases are likely to be identified in the coming days, including more cases in the U.S. Its also likely that person-to-person spread will continue to occur, including in the US. Widespread transmission of the COVID-19 in the US would translate into large numbers of people needing medical care at the same time. Schools, childcare centers, workplaces, and other places for mass gatherings may experience more absenteeism. Public health and healthcare systems may become overloaded, with elevated rates of hospitalizations and deaths. Other critical infrastructure, such as law enforcement, emergency medical services, and transportation industry may also be affected. Health care providers and hospitals may be overwhelmed. At this time there is no vaccine to protect against COVID-19 and no medications approved to treat it. Non-pharmaceutical interventions would be the most important response strategy.

When #COVID19 hits the US:

People without paid sick days will go to work sick

Sick people with copays & deductibles won't get tested because of $$$

People without insurance won't seek treatment because of $$$

China has performed nearly 200,000 tests.

South Korea has performed nearly 28,000 tests.

Meanwhile, US--with most expensive, but most dysfunctional, healthcare system on planet--has performed just 414 tests and has little or no capacity to perform more (https://cdc.gov/coronavirus/2019-ncov/cases-in-us.html)

Only three US states can test for coronavirus, says public lab group

California, Nebraska and Illinois

are the only U.S. states that can currently test for coronavirus, the Association of Public Health Laboratories (APHL) told Reuters on Friday.

The U.S. Centers for Disease Control and Prevention (CDC) last week said some of the testing kits sent to U.S. states and at least 30 countries produced "inconclusive" results due to a flawed component.

The CDC planned to send replacement materials to make the kits work.

Not normal- 2/3rds of all critically ill relied on ventilator machines. Moreover, “Severe Covid-19 cases spent more than 4 weeks on ventilators”. This isn’t like most viruses. Can other countries handle that kind of healthcare load?

This

  • I’ve been pointing out for 2 weeks that patients seem to be discharged (recover) very slowly and have very long lag before dying if they do. Thus if we see #COVID19 deaths in a country right away, we know it’s been spreading for likely a while. That’s unsettling to me.

There's a 9 day lag between onset of symptoms and progressing to severe pneumonia (for the approx 20% who develop severe symptoms). My guess is Iran are only catching the severe cases. There's possibly 100s of mild cases progressing to 1000s in the community

We should be shutting down air travel to several countries now or the United States is going to continue to be seeded with the virus!

CoronaVirus NEWS SUMMARY:

  • Outbreak in Italy worsens (152 cases in 3 days)
  • S.Korea raises red alert
  • Iran outbreak may be worse than thought
  • Pakistan and Turkey close borders

Just as China appeared to be slowing down, the past 3 days have taken us into new territory.
