Some sort of fault caused the entire metal fan to become electrically charged. When he grabbed it it was basically the same as sticking something metal into the outlet. The electricity causes his muscles to seize and he can’t let go, so he eventually fries.
I think the other guy legitimately didn’t see what was happening. As the video starts he’s already packing up his bag and his head never angles as though he’s looking toward the dying guy
I've gotten a shock from a metal-caged fan before. The coils are thin wire and the insulation isn't always great. Stick with three-prong fans and verify that the third prong really connects to the case and the outlet is really grounded.
A trick if you're paranoid is to touch the object first with the back of your hand. If it's live, your clenching hand won't grab the live object.
I remember reading about chinese electricians doing that. They didn't always have access to proper tools for testing current, so they just brushed it with the back of a finger to see if it was live.
