Windings get loose and touch chassis. More than likely the strain relief where the wires go into the fan have become undone, pulled, worn or frayed and are touching the chassis. But good chassis ground would still keep this from happening to this extent.
Plug had probably stopped being tested five years prior. Employees keep running over power cord with bags etc. Open current = bad day for that guy.
What do you mean plug being tested?
Test and tag. It's a common yearly test that companies get electricians in to test and inspect the plugs and extension cords then tag them as passed or need replacement.
Right?
