Don’t they check your name against the number on your card? And aren’t the penalties pretty severe for this? Anyone know anything about this?

Well theoretically there shouldn't be a database your employer has access to that has your medical records without your consent. But then theoretically you shouldn't lie to your employer.
If it turns into a government authenticated mark of the beast thing you are much better off having made your stand now instead of forging documents.
No and no.
Nobody has a way to check that is reliable. They aren't going to give a monkey at a venue gate that kind of information.
