The cause of the fire is being investigated.

Poor maintenance, pushing things too hard, and just not paying attention, the same reason there are a couple thousand derails a year in the USA.

Pretty much. Im sure there will probably be some government standards group chomping at the bit so they can fine the company for ignoring schedule maintenance or something.
