The backup mirror made by another contractor was made to the proper specs. Since no proper testing was done during assembly due to the testing facilities being under Air Force control and testing there would make Hubble a military project, the bad mirror wasn't discovered until after deployment. Had they tested it, the backup mirror could have been installed and worked as intended.
