It's the dumbest greeting. People don't want to know and no one gives an honest answer. This greeting confuses foreigners who think it is a real question.

"How do you do?" "Ok, where to start ..." <foreigner gives a five minute executive summary of his life>

I ususally reply with "cant complain. nobody listens to me anyway".

Where I'm from we often ask people we know how they are when we meet them and we expect them to talk and complain about how things are going in their life, give advice if possible.

Maybe you people should stop asking others how they are if you don't care about them.

Yes and yes.

I actively refrain from using this greeting.

Maybe those particular foreigners are especially retarded because I'm fairly certain variants of "how are you" and "how's it going" are standard greetings across many different cultures. Still is a silly greeting regardless. I think it's just a "pretending to care" formality that's been around for way too long.
