It's more than 1 in 1000. I go through a LOT of new FMJ from reputable manufacturers (Sellior & Belliot, Blazer, PPU, etc) and see at least a couple failures in each case.

Pretty smart actually
One of the most useful bits of training I've ever had was malfunction training. Instructor walks around and randomly taps you for a malfunction. Sometimes 1 in 50, sometimes every round. Got really good at tap-rack-bang by reflex.
You might want to try Winchester. It burns cleaner too.
