I don't disagree I wouldn't want mandetory medical in the west because there's no way to keep from turning it into a monster. I'm first and foremost a pragmatist and we can't have it, but in an ideal world some common sense things like, "No, it's not okay to be obese, you're a drain on society," would be mandated. But I like to say I'm the only honest liar in the room, and I'm a maximum freedom guy only because I'm (or someone who would do things as would be ideal) aren't absolute dictator, so absolute personal freedom is the next best alternative. All that said, Japan's attitudes about obesity are not just about their mandetory physicals. Japan (and much of east Asia) have absolutely no problem with telling fatties their fat and making fun of them for it, which is most of what the video is about.
