That is called a Panniculus and aside from some rare conditions or as a complication from pregnancy, it is something that only fat people get. When fat people get it it is called a Panniculus morbidus and having one is pretty much the definition of "morbidly obese." This is what is categorized as a Stage 1 Panniculus. Stage 2 is when it hangs with a crease like a fat roll. Stage 3 is when it hangs low enough to obscure the genitalia.
Edit: the stages are different than i remember:
- Grade 1: the panniculus reaches the pubic hair but not the genitals
- Grade 2: the panniculus lies over the genitals down to the thigh crease
- Grade 3: the panniculus reaches down to the upper thigh
- Grade 4: the panniculus hangs down to mid-thigh level
- Grade 5: the panniculus reaches the knees
