If the death rates are higher in Iran, it's likely from the average age being high. From what I've read, under age 65 <1% fatalities. Above age 80: 18% fatalities. Of course, these are early numbers and, like the Iowa caucus, may be inaccurate.

I'd bet that lack of a 1st world tier healthcare system will have an impact but with the Parliament that shouldn't be an issue. Just age and personal hygiene.
