The big problem is ass-in-chair managers that have no fucking clue how to manage a team properly and the ones that don't want to do it over digital com's because they are abusive fucks (recorded meetings, emails, etc).
"Middle management" is only needed for people that only do their job if forced to by a manager on a daily/weekly basis. Get rid of them, as well as the middle manager.
