If a drug gang threatened to destroy a shop if the owner didn't follow an arbitrary rule "no customers wearing blue" it wouldn't matter if there is no official legally installed law, the thugs' rule become the law of the land. The shop owner cares about reducing his personal financial risk and liabilities. Turning away the occasional customer to keep the gang happy seems to be the path of least resistance and the best choice for risk mitigation.

also, when they balk at your exemption claim - ask to see the manager. If the manager balks, ask them to write down the date and their name, contact # & signature on paper indicating that they are are discriminating you from service today on the premise of medical circumstance and/or religious beliefs.
Municipalities are also setting laws, The store owner gets fined if they don;t force masks on customers.

Don't pay them. There's no legislative authority behind the fine. You aren't a business. They can't go through the all too often attempted - and eventually thrown out - manner if revoking your (((license))) to operate.
A fine is not means for probable cuase. Not having a means for probable cause means you don't have to talk to the police. Means they can't arrest you. Means they can't search you.
Last time I was out, 90% of people walking in the open air were wearing masks.

I live in an extremely high class, liberal area. one of the top 3 wealthiest counties in my state. The most I see if someone is "in open air" is them having the mask on but pulled down around their neck for ease of putting it on if they need to enter a cucked store.
I live 3/4 a mile from the post office. I was in it just a few weeks ago not wearing a mask. One groid started to try and speak up to me and I just looked it in the eyes and said one word "no". The chimplips flapped no longer.
And soon they will have staph or strep bacterial pneumonia...Something that is actually dangerous. Just saying... that's what masks help promote.
At the Post Office I use the inner drop boxes after hours so I walk in without a mask on.
I've stopped shopping at most chain stores because of this bullshit. This has really shown me how many people out there are literal brainless NPCs.
You’re assuming people know, or give a shit about federal law compared to “mask mandates.” Cops aren’t going to enforce the Covil Rights Act over an “emergency” mask “mandate.”
