Large transformers can be monitored for developing failure.
1) sample the oil in the unit. Degrading materials, cellulose if I recall, start to build up as the unit ages.
2) measure and release building pressure in the oil tank. Oil can be turned into gas (maybe by electrolysis), as it degrades.
You need to change the oil every now an then, and these devices do wear out over time. They can be rebuilt more easily if you do not allow it to become a bomb rupture, leak, and burn.
Also, this may be an old transformer full of PCB. We'll see how big of a mess that is now.

Hoover Dam has been running and producing electricity for over 80 years. These turbines have had major overhauls several times. How did they get to the point where the maintenance was so poor that a turbine caught fire?

Explosion? Looks more like smoke billowing from an equipment fire. Notice there isn't any scattering of smoke to the sides or any disruption on the water from shrapnel/debris coming from the explosion source. Is there any video of an actual explosion or just this smoke rising?

Maybe that barrel at the base of the fire like it's tar burning. It's so coincidental they had the camera right there at the right spot to catch the smoke but no debris is shown at all. It's just the government making us go back to the stone age again so we'll appreciate all our modern conveniences.
