So many questions , but - Who was the person that issued the name tag?. Seems being issued name tags was for the schools vaccination event? The child whose name tag it was, had already been vaccinated earlier that day - but they didn't mark off the name tag or separate it out as vaxxed ? How were the name tags verified to the child ? Unless both children had the same exact name (and even if they did) then the individual giving out the name tags and not verifying them correctly with each child should be getting the short rope.
