Based on what we know about the later years of the Reagan administration, there is a good chance many of the decision are being made by Jill Biden. Several parts of the government are on autopilot like the DoD and CDC/NIH. The main problem is that information goes to Joe, which is just a black hole; but very little information goes to Jill. However, it is likely Jill who is telling Joe what to say and do. Many leaders in the executive branch agencies then just follow Joe's orders because all they really care about is keeping their salaries and their pensions.
