The entire movie was filmed in Ohio, and the train derailment scene was filmed in East Palestine, Ohio. Haven't watched the movie but the synopsis on imdb says this about the protagonist in the first sentence.:

Or was the movie adaptation just used to plan out the actual event?

Every fucking time there is a movie, crisis event, etc, it come true a few months later .
