Most people just see the bill, and pay it if it matches the SoB from their insurance. Call and force the hospital to bill appropriately. Quote your coverage policy at them.

Just got hit with a 3k bill labeled as surgery then when insurance reached out to the doctor, the doctor claimed it was for everything under the sun. When we were able to show we paid for every thing she mentioned already, she finally settled on 3 telehealth sessions. I asked why it was billed as surgery, she said it was just how they bill it. I told her it sounds like fraud to me. It's still ongoing. Fucking assholes are redpilling my wife. She's already on board but the more they do the more they redpill.

It is fraud. And they expect that most good White folks will just pay it.
