I have a very keen ear and have been friends, acquaintances and roommates with drug addicts in the past. I can tell someone is a serious addict just by hearing their smokey, gravelly voice on the phone. If they have a somewhat scratchy monotone gravelly voice, then It's almost a dead giveaway that they're a piece of shit junkie - mostly from heroin. Or maybe I just have a very sophisticated and advanced radar for bullshit.

Sounds like an alcoholic. There was a couple like that in one of the apartment buildings I grew up in - A skinny man & woman who were always screaming at each other over trivial things - because they were drunk all the time. They looked anorexic probably because they spent all their money on booze. All the other people and families in our building hated them because they caused so much ruckus towards other people in the building and neighborhood. My family moved out because we couldn't deal with their fighting and BS almost every day.
