well... I don't know for sure, but the bug that came up today regarding rate limit error messages, and AOU's response to that issue, indicates that it keys off of IP address and nothing else. The mere fact that some users see this is proof that some amount of traffic gets aggregated by ISPs via a NAT (this was AOU's assertion) ... and if this is the case then Poal servers see the traffic all coming from the same IP and can erroneously rate limit.
AOU and I were posting back and forth on this in a slightly adversarial way, but as soon as I said "hey if you use the same logic in the algorithm then it coudl throw false pasitives." BOOM ista-ban
