Looks like it failed to separate causing the spin's inertia to overpower the available thrust to control the spin. I'm assuming they destructed it.

I was surprised to see that they lifted off with apparently 3 or 4 unlit engines, and was also surprised to see that some engines were lost then were re-ignited midflight. It did not achieve orbit but it got the older hardware off the OLM to make way for the newer iterations already waiting in the wings.
