Are you sure you have your staging correct and that your engines have oxidizer available as well as liquid fuel while in a vacuum? When you're having problems, check the status of the engine and see if it says flame out. On your stage 1, what is lv t30 exactly? I haven't booted the game up in a while. You do have a control module of some sort, either a probe or a pilot right?

Do you mean re-entry? You might need a heat shield. Vacuum engines are incredibly inefficient below 40-50km.
