This guy had to climbed over something. This isn't the 70s, that tub was guarded. I've managed manufacturing, the only way to get insurance is to be involved with osha's safety measures.
I'd bet a dollar that there was handrails there and he was supposed to be tied in to a cable. Maybe nobody ever told this dude about it. It's hard to fix stupid, this guy apparently was.

No they didn't have appropriate railing or fall arrest equipment. Working in that environment is still really stupid though.
