sounds like he has something that might have acid on it and it is soaking into his skin, he's using meth but doesn't realize what it does since if it's real far right as in hardcore bikers they might be giving it to him since they might have a ton of it around, last he got the injection and it's stimulating the paranoid part of his brain or he's not getting rem sleep since insomnia is one of the side effects and his mind is not defragging like a PC and that is what dream state does is sort the daily information but if he's gone like over a week on 2 hrs of broken sleep it would lead to this also.
