Because quality of ingredients and methods of cooking in fast-food industry have nothing to do with producing healthy food. Pretty opposite. Drenched in oil, over salted, over spiced, containing cosmic level of calories.

If you buy ingredients for your burger from local farmers, fresh vegetables, fresh high quality meat then there is none reason to call that food unhealthy.

But mostly healthy/unhealthy has to do with amount of calories that you spend. If you are working desk job and eating mcdonalds burgers with fries twice a day, then its not great for your health. But if you are training every day to look like some comic-book hero who is ready to participate in upcoming race war, then you need these calories.
