Ground team members and Management and Support Staff: Submit here. You can use your AA ID to sign in.

U.S.-based mainline flight attendants: Complete and submit the form in the Comply365application on your company tablet, or go to aa.comply365.net and navigate to the Forms section on your personal computer or mobile device. U.S.-based mainline pilots: Fill out the form in the Comply365 application.

It’s not too late to be vaccinated.

To be considered fully vaccinated, you need to have the full course of doses for your vaccine — that means one dose of Johnson & Johnson or two doses of the Moderna or Pfizer vaccine. If you haven’t been vaccinated yet, there’s still time to get the Johnson & Johnson vaccine by Aug. 31.

Not sure how to get a vaccine appointment? Here are a few ideas:

Talk to your doctor. Several doctor’s offices now offer the vaccine, or your physician can refer you to a local vaccination site. If you don’t have a doctor, Accolade (for team members enrolled in the Core, Standard, High Cost Coverage, PPO 80 or PPO 90 plans)

and the DFW ConnectedCare Center (for team members enrolled in the DFW Connected Care medical option) can help. Check with your local pharmacy. Drug stores like CVS and Walgreens are among those offering the vaccine. Go online. The Centers for Disease Control and Prevention site has a helpful list of locations. You can also visit your local government’s vaccine website.

First they came out against it now they are pushing it let’s see if Delta & Southwest follow through.
