One little piece of programming would end it all. Verizon, AT&T, etc. do not want to end it because they want to sell you services to block them.
All that's required is to charge callers whose caller ID doesn't match their ANI $0.001 for each call. Even if you used your Google Voice number to make 1,000 calls a month you'd be charged $1. Scammers dialing millions of numbers would have to pay thousands.

This is the correct answer. I've worked for both companies in my career and the fact is that every single decision is about driving profit. Customer service is a distant second.
