I was told to avoid science completely. Don't mention safety, efficacy, ingredients, etc. Don't be combative - don't spew laws at them or be snippy. Don't be too wordy or terse (I know); I answered the questions with a sentence or 3 and tried not to give them any loopholes. If you mention fetal cells be very careful. They were used in the research but not necessarily in the injection. I avoided that altogether just in case another comes out that claims they didn't use them. Avoid mentioning any specific religion. To be religious you do not have to go to church or have a pastor sign off on you - you just have to have sincerely held beliefs.
I didn't lie at all in mine and I've not been to church since I went with a friend in middle school. I have plenty of spiritual beliefs that may not line up with any mainstream religion and I don't believe organized religion is for me in the least, but I still got the exemption (I was vague enough in explaining that they probably assume I'm some fundamentalist sect). I essentially said I don't pollute my body with crap and I trust in my faith to keep me well.
Honestly, the whole situation made me see my personal beliefs in a whole new light and I would no longer hesitate to call myself religious. I still find it absurd that I need to explain myself at all and they better keep what I said private. I know they told my manager about the exemption but so far he's been professional enough not to bring it up (my old manager as of 6 months ago probably would have found a way to out me on a team meeting which would have made for a hostile work environment to say the least).

Do you happen to have an example version of your submission, that others might be able to use? I found out earlier this week that I need to submit something by this weekend, so I've been stressing out trying to get something together that will be approved. I saved this thread for later reference, but I didn't expect to need it so soon.
