For a sin to be a mortal sin it must meet three conditions:
- It is a sin of grave matter.
- It is committed with full knowledge of the sinner.
- It is committed with deliberate consent of the sinner.
Once saved always saved is made up nonsense. It's basically universal salvation. If you believe in "once saved always saved," then you would think Judas is in heaven, but Christ said it would of been better if he was never born.
