In the medical field if someone is self respecting and does their job correctly - informed consent is precisely that - you have the right to refuse if you feel that the risk outweighs the benefit, or if the procedure itself is not worth doing. In people who are young and healthy there is no need to do said procedure (because there is no benefit and only risk). In the old they have the right to decline if they wish, even if said declining eventually results in their death (much like what many cancer patients will do - they decline treatment because they feel its futile or because they don't want an impaired quality of life). If it were really about health - we'd subsequently ban the obese from restaurants, alcoholics from pubs and clubs, and those with an STD from a nightclub or brothel.
