Well, if you are automating a heated tank for example, it would be a good idea to have a physical or at least a PLC failsafe in addition to your Linux control system to shut off the heating elements if the contents explode above a certain temperature.
If there is just Linux standing in the way of death, you're doing it wrong, and if you are a P.Eng. I expect you would lose your license if there was ever an accident.

One would like to hope people that make those mistakes lose their license but it isn't always the case. I know of a few specific devices that do not have that type of redundancy. Nothing has happened as far as I am aware yet but that doesn't mean it can't
