I don't have much info on the subject but my assumption for the "Permission" part is that maybe with assisted suicide, the government may allow differences for insurance payouts etc... Whereas an individual who just kills themselves might not pass anything onto their beneficiaries? Just tossing out straws. I do agree though, something so basic does not require a permission slip in general. It's probably rooted in financials would be my guess.
