I did that a few times, and I only put initials on the forms.
Turns out that everytime the run a sample with the DNA unable to be linked to a name or ssi #, its unreadable and the sample is "corrupt"

It doesn't say they are a scam, says you are giving your genetics to nefarious people sometimes.
