When your relatives are in the hospital, especially when they need assistance, visit them every day. Long ago my grandmother was in the hospital and the nurses put food in front of her and then took away the uneaten tray. She needed assistance to eat. We caught it in the first day and visited each day after. Don't trust hospitals.

Exactly this. Also make sure you know what medications they are giving and when. Over 75% of deaths in long term care are human error and preventable.
