This has been happening for at least two decades. Doctors did it to themselves by supporting the initiatives in the 80's and 90's. Now, medicine in the US is run by insurance companies, and hospitals are being acquired by massive healthcare corporations, and you never see an itemized bill.

There should be a law preventing healthcare service providers from communicating with insurance companies. Costs should be up front and bills itemized and sent only to the patient. This would solve a lot of the issues we have today.
