Shawn Baker, MD – License no. MD2006-0659 – Stipulation and Order for Voluntary Surrender of License and Permanent Restriction on Reapplication was issued accepting Dr. Baker’s voluntary and permanent surrender of license stating that he may not petition the Board for reinstatement, renewal, restoration or reapplication for any license to practice medicine in NM without first proving clinical skills competency. This action was based on failure to report adverse action taken by a healthcare entity and incompetence to practice as a licensee. (September 2017)

Doesn't that indicate he gave it up voluntarily, or is there more to the story here I don't know?

I believe he's trying to get it back.

This phrasing may be similar to "He decided to leave the company"

Then isn't it a little disingenuous to say it was revoked?

Possibly, but what I think the source means is that he had to give it up, but did so without fighting
