Ball died on April 12, 2001, aged 79, as a result of liver failure following a short illness.
I can't find anything on the cause of the liver failure. It could have been due to a number of things, but one possibility is alcohol consumption. However, I don't know if he was even a drinker - just noting it as a possible cause as it would be ironic to die of alcohol-induced liver failure from being an alcoholic while simultaneously being the designer of the iconic Smiley Face.

Doesn’t have to have the bloated liver of an alcoholic in the pic. Hard to say, but a happy man he does not look to be.
