In Africa, they eat mostly food that grown there, fish there and their mamas cook from scratch every time they have a meal. The malnourished Ethiopian kid is an old trope that isn't common at all. As for the diseases they are all endemic to Africa for the most part and you could argue that these natural hurdles make for a good natural selection.
