This one I'm betting was sheer incompetence. Cocoa powder can blow up when aerosolized, just like flour or corn meal. Get the right powder/air distribution, and you have a thermobaric bomb. Some incompetent didn't pay attention in training, and then ended up not training the asshole who blew up the factory.
